Certified Laboratory Quality Specialist CLabQS (HKSQ) Registration Who are eligible Individuals who have fulfilled HKSQ requirements stated herein, and are members of HKSQ in good standing, are eligible to apply for registration. On approval of the registration, a certificate will be issued to applicant and the registration needs to be re-certified on a tri-annual basis. How to be certified (A) By successful completion of the certificate course entitled "Quality Management for Laboratory" offered by HKSQ Individuals who had attended the certificate course on Quality Management for Laboratory and passed the examination and with 3 year related laboratory experience*, can apply for registration as Certified Laboratory Quality Specialist (HKSQ). (B) By Exemption Individuals, who had, in previous 5 years, successfully completed a course accredited by HKSQ¡¦s Accreditation Board and with 3 year related laboratory experience*, can apply for registration as Certified Laboratory Quality Specialist (HKSQ). In both cases (A) and (B) above, the applicant has to be a member of the HKSQ in good standing when he/she applies for certification registration. A HKSQ Certificate will be awarded to successful applicants and they will be automatically registered for 3 years as Certified Laboratory Quality Specialist (HKSQ), or CLabQA (HKSQ) in abbreviation, and their names will be listed on HKSQ¡¦s website. * The 3 year related laboratory experience should be verified by company letter(s). Appication Form How to be re-certified Certified Laboratory Quality Specialist (HKSQ) whose registration will expire in 6 months¡¦ time can apply for re-certification. Such applicants must show and prove to the satisfaction of HKSQ that they have work experience associated with the laboratory quality process in the past 3 years and joined HKSQ activities as CPD in order to be re-certified.
A CLabQS (HKSQ) who is not re-certified before the expiration date of the registration will be de-registered. Applications for reinstatement of the qualification are regarded as new applications for registration. Benifits
